I have been tasked with reclassifying our inmates and am looking to find a better format. Our current form is basically our initial form with 3 questions regarding institutional history. I am looking to see if any jails have either paper versions or fillable (excel, adobe) reclassification forms.

Joel,

All reclassification are via our Jail Management System. Basically, we have 9 questions:

Medical observation or history

Mental Health observable or history

Suicidal observable or history

Victimization history or observed propensities

Current or history of behavioral issue

Assaultive / sexual charge or history

Current charge(s) high risk offender

Escape history from secure facility

Under the influence drugs/alcohol

This is used at time of booking and if reclassification is necessary.
