Who is doing MAT/MOUD, and what type they offer? One of the policies that I am looking at here is ACH is discontinuing suboxone when individuals have holds in other counties. I think it would be better practice if we could keep folks on it if they are going to a county that also does MAT/MOUD, just like other medications that go with them. I can put a simple google form together too if you think that would be helpful.
I just want to be able to compile a list of each county and what type of MAT/MOUD they use (Suboxone, methadone, Subutex, etc.)
Thanks!

Washington County does something similar. If they have holds from other counties that allow suboxone they will continue to give it to them. If they have holds from counties that don’t provide suboxone they will not give it to them. I have no idea what kind of suboxone the nurses hand out. It is run by our medical department. I believe they may already have a list if that helps. lindsay.mielzarek@co.washington.mn.us is our nurse supervisor (she is out of the office until 12/20/23 rolando.verajr@co.washington.mn.us Will also be able to provide that information.
St. Louis County has an active MAT program. Our provider will continue suboxone or methadone if they come in with an active prescription, but we don’t start anyone new on methadone. We will get people started on suboxone with a rule 25 and identified MOUD diagnosis along with a collateral source. We have also been looking into starting Subutex due to a problem with diverting but those details have not been finalized at this point.
Scott County
This is from our Lead Nurse, Laura Perkins, “We do not have access to methadone. If a person brings in a supply of their own or we can verify suboxone/Subutex we continue. We review each person’s medical chart, court information, and discharge (release to other county, treatment, or to the streets) before deciding to restart. As for a person never been prescribed we follow a process of them seeing mental health, the medical provider and the social worker before starting them. If you need me to further explain, please let me know as it is mainly patient specific how we proceed.”
